Most of our questions have the option not to answer if that is what you choose, but so you know what to expect when you complete either of our forms below is a list of the questions to expect.

The majority of the questions on our Speak to an Adviser and Access Support form are multiple choice, with a free text box where you can tell us about what happened if you choose to do so. You will be asked to provide your name and contact details so an Adviser can contact you.

In the Anonymous Reporting form, all questions are multiple choice. You will also be asked about your reason(s) for reporting anonymously. Please remember we are unable to take action on anonymous reports.

Does your request for support concern gender-based or sexual violence, or does it concern bullying and harassment? (we include some examples of these to help if you aren’t sure)

Do you feel any of these factors contributed to the behaviours (protected characteristics as defined in the Equality Act):?
